If you have written records of a physical exam /TB test performed within the past six (6) months, such records will be sufficient. If not, a local healthcare provider will conduct the physical, tests and drug screen at the expense of Bethel Bible Village.

Sign a release authorizing Bethel Bible Village to accomplish background checks as necessary.
